Family Based Care Tasmania offers low level support services under the Commonwealth Home Support Program (CHSP) to people living on the North West Coast of Tasmania. If you live outside of this region please contact us to find out about other options that are available to you.
This Program is a Government subsidised Program for people over 65 who need help to continue living independently at home and in their community. Our focus is to help you with the tasks that you find difficult and to support you to continue with those tasks you are capable of doing yourself.

If you have been injured or hospitalised you may be eligible for extra services through the program for a short time to help you get back on your feet after you return home.
Call My Aged Care on 1800 200 422. The My Aged Care staff will ask you questions about your current needs and circumstances and arrange a home support assessment with a Regional Assessment Service (RAS) before you can begin receiving care.
Our Care Coordinators will visit you at home to help you work through the My Aged Care process.
The Australian Government expects you to contribute to the cost of your care if you can afford to do so, however the fee you pay will depend on your individual circumstances.

Wellness emphasises identifying needs, aspirations and goals. It acknowledges and builds on strengths and has a focus on integrating support services as a path to greater independence and quality of life.
Reablement emphasises assisting people to regain functional capacity and improve independence. Similar to rehabilitation, it is goal-oriented and aims at full recovery where possible – it seeks to enable people to live their lives to the fullest.
