Home Care Packages

Family Based Care North West is an approved provider of Home Care Packages. We provide support services across Northern Tasmania including King Island and the West Coast and have offices in Burnie, Devonport and Launceston.

What is a Home Care Package?

A home Care Package includes services that help you live independently in your own home for longer – you choose when, where and how you need support.  Our services include, but are not limited to the following; house cleaning, laundry, gardening, home modifications and maintenance, cooking, showering, dressing, personal hygiene, social activities, transport, and companionship.

Your Case Manager will work with you and/or your Advocate  to develop a care plan that suits your individual needs.  You will receive a copy of the agreed care plan which can be altered at any time if your circumstances change. Formal reviews are also conducted annually to ensure your support needs are being met. In addition, our experienced and friendly staff are available for you to talk to as required.

Support is available every day of the year.

Are you eligible for a Home Care Package?

You may be eligible for a Home Care Package if you are:

  • an older person who needs coordinated services to help you to stay in your home
  • a younger person with a disability, dementia or other special care needs that are not met through other specialist services.

Our Pricing

We will work with you to come up with a package that meets your budget. We do not charge entry or exit fees and are transparent and upfront.
